HI KAYLA,
I DO NOT QUITE KNOW HOW TO GET WHAT I NEED FROM AQHA. THE AUTHORIZATION FORM DOES NOT COVER WHAT I AM LOOKING FOR. MY HUSBAND AND I OWN A HORSE. THIS HORSE HAS BEEN SENT TO VIRGINIA WHERE SHE IS BEING TRAINED BY MY DAUGHTER. EVENTUALLY, MY DAUGHTER WILL SHOW THE HORSE AND MAKE ALL DECISIONS ABOUT HER SHOW CAREER. I NEED A FORM THAT WILL ACCOMPANY MY DAUGHTER TO EVENTS; AND WILL LET HER ENTER SHOWS AND MAKE DECISIONS ABOUT THIS HORSES FUTURE WITHOUT MY HUSBAND I BEING PRESENT.DO YOU HAVE SUCH A FORM ,OR WHAT CAN WE LEGALLY DO TO HAVE AQHA RECOGNIZE OUR WISHES? PLEASE ADVISE US

Michele,

AQHA will change your name and/or address on file. To change your name and/or address on your horses’ certificate, please return the certificate along with the $10 correction fee for each certificate. Changing the name on the certificate is not mandatory. (AQHA and/or Show Management may require a copy of the marriage license or birth certificate as documentation of ownership.)

AQHA does not have a form to cancel an authorization. A signed statement submitted to AQHA will suffice.
